   Inspection/Sting Report

   Minor In Possession of Tobacco Monthly Summary Report

These forms are submitted to the Comptroller’s Tobacco Enforcement Program by the 10th of the month immediately following the month in which the activities occurred:

 

   Cigarette and Tobacco Products Retail Employee Notification

By law, Cigarette/Tobacco Products Retailer Permit holders are responsible for the training and supervision of employees involved in the retail sale of cigarettes and/or tobacco products.

This form applies to all Texas Cigarette/Tobacco Products Retailer Permit holders to document their compliance with Texas Health & Safety Code, Sec. 161.085, which requires retailers to notify their employees and agents of state law regarding the retail sale of cigarettes and tobacco products. 

Download and print or photocopy and use this form to provide the required notification to each employee within 72 hours of when the employee begins selling tobacco products at retail.  The retail employee must sign the form as proof that notice was given.

   Texas Cigarette/Tobacco Final Proceeding Report

This form applies to all Texas courts of jurisdiction, usually municipal and justice courts, for violations of Texas tobacco laws. 

The court should report all cases with final disposition in each month by the 10th of the following month to the Comptroller's Office.
